Add our complicated health care environment, tepid economic growth and a rapidly aging population, and it is even more daunting. One factor is certain, however: Health care organizations must work together more closely to achieve better coordinated care, a more efficient payment system and improved outcomes for patients. Patients are demanding it, and most likely the Affordable Care Act, in whatever form it takes, will require it. Innovative approaches to care such as the Patient Centered Medical Home are exactly what the health care system must adapt to ensure better care and outcomes, no matter the state of the economy. To continue the metaphor, the PCMH can make crystal-ball gazing a less uncertain proposition.”
